• Assisting individuals with acquiring, maintaining or improving skills through their participation in a variety of everyday life activities, including outcome based Activities of Daily Living Goals, volunteer and educational activities.
• Helping consumers locate desired activities that are necessary for them to live in the community with an increased sense of independence and productivity.
• Supporting individuals in a manner that ensures the person’s health and welfare.
• Supporting individuals in accessing and using community resources such as instruction in using transportation, translator and communication assistance.
• Assisting consumers with non-outcome based companion services for activities such as shopping, medical appointments, and other activities.
• Supporting individuals in developing financial stability and security, such as assistance in arranging for disability-related work incentives and plans for achieving self-support; general banking; assistance in beginning a business enterprise; personal and estate planning; balancing accounts; preparing income taxes; and recordkeeping.
• Supporting individuals to participate in community projects, associations, groups, activism and advocacy.
• Providing support that enables an individual to visit with friends and family in the community.
• Supporting individuals who participate in public and private boards, advisory groups, and commissions.
• Assisting individuals in exercising their rights as a citizen.
• Providing support that enables the individual to benefit from the participation of friends, relatives, and advocates as part of the individual’s support planning team.
• Supporting the individual with meeting the goals established with other members of the team through collaborative and consultative meetings to provide feedback on the consumer’s progress.
